All new patients need to complete new patient paper work which can be downloaded online - here- so they can be completed in the convenience of your own home or office, before you even arrive to Neurology Associates. OR new patients can come to office 30 minutes before appointment and our staff will provide you printed copy of paper work to fill out.
Please bring the following information with you:
- Completed New Patient Paper work (only for new patients).
- Current insurance cards.
- A current photo I.D. (driver license).
- List of current medicines and their dosages.
- If referral is needed, please bring referral paperwork.
